In this role, you will provide essential care to a high-acuity patient population, including those recently discharged from the hospital, ED, and walk-in clinics. Your expertise will be crucial in preventing readmissions and empowering patients to heal in the comfort of their homes.</span></p><p></p><p><span>This is a chance to step into a fast-growing and vital field. As virtual care continues to evolve, so will your role—offering you a unique opportunity to grow and innovate alongside us.</span></p><p></p><p><span>You will provide care to patients in their homes using telemedicine as part of a team-based model of care which will at times include community paramedics. </span></p><p></p><p><span>You can either work remotely or in a hybrid model.</span></p><p></p><p><span>You must be board certified/eligible and licensed to practice in New York State.
