Role Overview

As a Part-Time Admissions Nurse, you will:

  • Complete comprehensive OASIS assessments accurately and efficiently.
  • Admit new patients into Home Health services while ensuring compliance with CMS guidelines.
  • Provide skilled nursing visits and chart same day to maintain quality and timeliness of care.
  • Collaborate with the interdisciplinary team to establish individualized care plans.
  • Educate patients and families on home health services, medications, and care expectations.
  • Represent Summit with professionalism, empathy, and accountability.

This position is part-time and offers flexibility—ideal for nurses seeking additional income, a more adaptable schedule, or a balanced role alongside other commitments.

Requirements

  • Current Utah RN license (required)
  • Minimum 1 year of Home Health nursing experience (preferred)
  • Experience with OASIS assessments (required)
  • Ability to complete documentation accurately and on the same day as visits
  • Strong communication and organizational skills
  • Current CPR certification
  • Valid driver’s license and reliable transportation
  • Familiarity with Kinnser/WellSky (preferred)
